BACKGROUND
A 33-year-old man with HIV presents to the hospital with a fever, a cough, and shortness of breath that has lasted for 3 days.
Hint
The patient is not compliant with his medications. Blood gas measurements reveal a dissolved oxygen level of 64 mm Hg and a lactate dehydrogenase (LDH) value of 475 U/L.
