BACKGROUND
A 56-year-old man has a cough, fever, chills, and dyspnea that developed over 2 months. He received a course of macrolide and fluoroquinolone antibiotics, without improvement. The patient previously underwent mechanical aortic valve replacement because of aortic stenosis.

This patient has a history of ventricular tachycardia.
