BACKGROUND
A middle-aged man presents with progressive painless dysphagia of several months' duration. He has no other symptoms. His voice has not changed, and he has no aspiration or regurgitation of food. Clinical examination findings are essentially normal. A barium swallow is performed. What is the diagnosis?
