BACKGROUND
A 46-year-old man is referred to you because a chest radiograph incidentally depicted poorly defined nodular opacities in upper zones of the lungs. This man is a nonsmoker and entirely asymptomatic. He has a history of renal transplantation and is currently receiving immunosuppressive therapy.

High-resolution CT shows bilateral fluffy areas of ground-glass attenuation in a nodular configuration. Concern regarding an opportunist infection prompts further investigation. Radionuclide imaging studies of the thorax reveal the cause of his problem.

What is the diagnosis?

The imaging findings are more than 100 HU on the Hounsfield scale
